Overview
The NAPLEX (North American Pharmacist Licensure Exam) is divided into five main content domains, covering Foundational Knowledge, the Medication Use Process, Person-Centered Assessment & Treatment Planning, Professional Practice, and Pharmacy Management & Leadership, with the largest portion (40%) focusing on patient care and treatment plans. The exam uses ~250 questions, but only 185 are scored, with the rest being pre-test items, and it tests your ability to apply pharmaceutical knowledge across these key areas.

If you’re a pharmacy student getting ready to take the North American Pharmacist Licensure Examination (NAPLEX), you’ve probably heard terms like “exam blueprint,” “competency statements,” and “content outline,” but what do these terms really mean? Well, put simply, these are tools that outline the areas of pharmacy practice knowledge that will be tested on the NAPLEX. An exam blueprint lays out the key elements of the exam, and competency statements and content outlines are 2 types of exam blueprints. Starting this year, we will transition the NAPLEX exam blueprint from the competency statement format to a content outline format. In this blog, we will breakdown the updated content outline to better understand the NAPLEX.
Key Aspects of the NAPLEX Exam
The NAPLEX exam tests your knowledge, ability and competence to perform pharmacy at entry-level. It is an examination that tests and evaluates every clinical corner of the profession.
Essence of the NAPLEX Exam
To practice pharmacy in the United States, you must pass the NAPLEX exam.
It’s not enough to have a pharmacy degree. Though a necessity, a pharmacy degree is just the first stage of becoming a pharmacist. The degree prepares and equips you with the knowledge needed to progress onto the next stage. The NAPLEX exam takes your knowledge of pharmacy one step further.
The NAPLEX exam has 250 questions. Some of these questions require a single answer, whilst other questions require you to select more than one correct answer. Some questions focus on the fundamental science of medicines, whereas other case study, or “scenario”, questions require you to understand, interpret and evaluate patient / clinical data.
To succeed at the NAPLEX test, candidates must possess exceptional knowledge of drug therapy and practice, dispensing and formulation science, and promoting public health and safe counselling procedure. As always, there are legal dimensions that candidates are expected to have a comprehensive knowledge of.
Here are the five content domains and their approximate weight on the exam:
- Foundational Knowledge for Pharmacy Practice (25%)
- Covers pharmaceutical sciences, compounding, calculations, drug development, and research.
2. Medication Use Process (25%) - Focuses on prescribing, documenting, dispensing, administering, and monitoring medications, including substitutions and immunizations.
3. Person-Centered Assessment and Treatment Planning (40%) - The largest section, dealing with patient histories, conditions, health assessments, and creating treatment plans.
4. Professional Practice (5%) - Includes public health, adverse drug event reporting, ethics, and social determinants of health.
5. Pharmacy Management and Leadership (5%) - Covers pharmacy operations, quality improvement, inventory, and leadership principles.
Exam Format:
- Total Questions: Around 250 (185 scored, 65 pre-test).
- Time: 6 hours for the exam itself, plus breaks.
- Scoring: Pass/Fail, with internal scaling based on difficulty.
NAPLEX Exam Eligibility
To be eligible for the NAPLEX, you must be a graduate of an ACPE-accredited pharmacy school or a FPGEC-certified foreign graduate, meet your chosen state board’s specific requirements (like internship hours/jurisprudence exams), and apply through the NABP to get an Authorization to Test (ATT). The process involves creating an NABP e-Profile, confirming your education, paying fees, and waiting for the ATT to schedule your exam with Pearson VUE.
